New Client Information and Check-List

Before We Get Started

  • Call our Intake/Office Coordinator at 404-228-8558 or email us at info@propeltherapyatl.com to schedule an initial evaluation or initial therapy session.

  • If you are using insurance, please have your insurance information ready. We will reach out to your insurance provider and email you your insurance benefits available for speech therapy services for you to make an in-formed decision about out-of-pocket fees and expenses.

  • Once scheduled, you will receive an email with our online intake paperwork. All paperwork must be completed by the time of your initial appointment. Please begin this process ahead of time as we will need a lot of information in order to best prepare to serve your child/you.

New Client Checklist

New Patient Forms are completed in the IntakeQ system

Prescription faxed to 404-228-8659 from you Primary Care Physician or your child’s pediatrician

Any current/past speech, occupational, or physical therapy reports are emailed to info@propeltherapyatl.com

Any current IEP is emailed to info@propeltherapyatl.com

Scan/Photo of the front and back of your insurance card is emailed to info@propeltherapyatl.com

Review your insurance benefits and determine if a pre-authorization is required
